We have a powered vent on ours, and a gasketed 
lid. The toilet is under negative pressure, so 
there is no smell at all, less than from a regular toilet.

We probably pay a small energy penalty for 
running a fan all the time, especially during 
very cold weather. We're actually considering 
hooking up a heat recovery ventilator to the 
stack, which would have the additional benefit of 
increasing the air changes in the house. I saw 
this done on a job on an off grid house north of 
Toronto, designed by Allen-Kani Associates:
