We have a Rinnai Tankless water heater and have the Qxygenics Low Flow 
Shower Head (2.0 gpm). In addition, we installed one of Dr. Vasile's 
GFX-Star S4-60 gray water heat recovery exchangers. Our drain water 
preheats water that is then fed to the water heater. We have never had a 
problem with the heater not providing hot water due to the use of the 
low flow shower head. A recirculation pump does seem like a good idea as 
pointed out by others. If you place any hot water heater a significant 
distance away from your point of use, you will have to wait for hot 
water to get to the faucet and you will suffer some loss of efficiency 
even if your pipes are insulated.
